Automatic placement needs a face in the photo. On a picture with no face in it the app adds nothing uninvited. Use the tray.
Drag it, pinch it to resize, or turn it with two fingers. With VoiceOver, select the sticker and use its Move, Larger, Smaller and Rotate actions.
Some stickers are drawn symmetrically, so flipping them would change nothing at all. The button is dimmed on exactly those and live on the rest.
Save writes it to your photo library. Share sends it wherever you choose. Both export at the photo's own full resolution, not a shrunken copy.
